Online Forms for Faculty

• Are you traveling? You MUST:
  • Complete the Request for academic leave – 7 days or less (online form)
  • Fill out this form to be covered by UC travel insurance
• Sabbatical Request Form – complete the online form, it will automatically be sent to staff.
• Financial transactions are now submitted via the ERSO (Shared Services Team 2) Intranet
